IUI vs IVF with donor sperm: which pathway is right for you?
Understand how each treatment works, what can influence the recommendation for you and how to plan your next steps with confidence.
Both IUI and IVF may be options for those considering fertility treatment with donor sperm – the most appropriate choice depends on your individual fertility health and circumstances. IUI with donor sperm may be suitable if you have open fallopian tubes, are ovulating or can have your cycle monitored, and there are no other known issues likely to affect treatment. IVF may be recommended where your health or treatment goals make it a better route. In this guide we explore how each treatment differs and how you can make the right decision for you.
At London Women's Clinic, we'll assess your fertility health and talk you through the advantages and considerations of each option before recommending an approach. Book a consultation with our fertility specialists for personalised advice on IUI vs IVF with donor sperm.
The short answer: the right route depends on your clinical picture
There isn't one donor sperm treatment option that's right for everyone. Your age, ovarian reserve, ovulation, fertility history, health of your fallopian tubes and any previous treatment can all influence whether IUI or IVF is better suited. Your plans for the future may matter too, particularly if creating and freezing embryos for later use is important to you.
These factors need to be considered together rather than in isolation. Fertility tests, scans and a review of your medical history can give your specialist a clearer picture of your reproductive health and help you make an informed decision about treatment.
IUI with donor sperm: when it may fit
IUI with donor sperm is a relatively simple fertility treatment in which prepared donor sperm is placed directly into the uterus around the time of ovulation. Depending on your circumstances, treatment may take place during your natural cycle or with medication to stimulate ovulation.
Donor IUI may be discussed if your fallopian tubes are open and healthy, you're ovulating or your cycle can be appropriately monitored, and there are no significant fertility factors that could reduce the likelihood of IUI being successful. Because fertilisation takes place inside the body rather than in a laboratory, at least one functioning fallopian tube is needed for the egg and sperm to meet.
IUI is generally less invasive and involves less medication than IVF, which can make it a preferred starting point for some people using donor sperm. However, it has a lower chance of success per cycle than IVF, and some patients may need more than one attempt. Your fertility specialist can help you weigh this alongside your age, test results and wider treatment goals before deciding whether to proceed with IUI.
When donor IUI may not be the right route
IUI may be less suitable when there are fertility factors that could reduce the likelihood of sperm reaching and fertilising an egg, for example, if both fallopian tubes are blocked or significantly damaged.
IVF may also be discussed if you are older, have severe endometriosis, a low ovarian reserve, or have had previous unsuccessful IUI cycles. Your fertility specialist may advise against spending several cycles trying IUI where IVF could offer a greater chance of success per cycle. Age can also influence your recommendation, as both egg quantity and quality decline over time.
But it’s important to remember that these aren't fixed rules. Your test results, medical history and previous fertility treatment all need to be considered before deciding when to choose IVF over IUI.
IVF with donor sperm: when it may be recommended
In IVF using donor sperm, the ovaries are stimulated with medication before eggs are collected and fertilised with sperm in the laboratory. An embryo is then transferred into the uterus, with suitable remaining embryos potentially frozen for future treatment.
IVF gives the clinical and embryology teams more control over the fertilisation process than IUI and makes it possible to create and freeze embryos. It may also be more efficient than IUI in older people. This can mean IVF with donor sperm is a better option for those at risk of having damaged fallopian tubes, endometriosis, concerns about ovarian reserve or previous unsuccessful treatment. It may also be considered when creating embryos for future use forms part of your family-building plans.
IVF is more involved than IUI, requiring ovarian stimulation, egg collection and laboratory fertilisation, and usually costs more per cycle. However, it generally offers a higher chance of success per cycle. Your specialist will discuss these differences in the context of your fertility health and priorities rather than recommending IVF based on a single factor.
IUI vs IVF with donor sperm: key differences to discuss with your clinic
The differences between donor IUI and donor IVF go beyond how fertilisation takes place. Medication, monitoring, cost and what you hope to achieve from treatment can all influence your decision.
|
| IUI with donor sperm | IVF with donor sperm |
| How it works | Prepared donor sperm is placed directly into the uterus around the time of ovulation, with fertilisation taking place inside the body. | Eggs are collected and fertilised with donor sperm in the laboratory before an embryo is transferred to the uterus. |
| Medication | May be carried out in a natural cycle or with medication to support or stimulate ovulation. | Usually involves ovarian stimulation medication before egg collection. |
| Monitoring | Cycle monitoring helps determine the right time for insemination. | More intensive monitoring is required during ovarian stimulation and before egg collection. |
| Invasiveness | No egg collection procedure required. | Requires egg collection and embryo transfer. |
| Cost | Generally costs less per cycle, although multiple attempts can affect the overall cost. | Usually costs more per cycle because of the medication, procedures and laboratory work involved. |
| Success expectations | Generally has a lower chance of success per cycle than IVF. | Generally offers a higher chance of success per cycle, although individual outcomes depend on factors such as age and fertility health. |
| Embryo creation | No embryos are created outside the body. | Embryos are created in the laboratory, allowing development to be assessed before transfer. |
| Future planning | Each attempt usually requires another insemination cycle and donor sperm sample. | Suitable remaining embryos may be frozen for future use, which can influence longer-term family planning. |
Neither option is automatically better. The purpose of comparing donor sperm IUI vs IVF is to understand how these practical and clinical differences apply to your circumstances, including the potential implications of needing more than one cycle. You can also explore London Women’s Clinic’s latest success rates, remembering that individual chances of success depend on factors such as age and fertility health.
Where ICSI fits with donor sperm treatment
ICSI (intracytoplasmic sperm injection) isn't a separate donor sperm treatment pathway, but a laboratory technique that can be used as part of IVF. Instead of placing eggs and sperm together to fertilise in the laboratory, an embryologist selects a single sperm and injects it directly into an egg.
ICSI may be discussed where there are concerns about sperm quality, including the quality of frozen sperm after thawing, or where previous IVF treatment has resulted in poor or failed fertilisation. There may also be specific embryology or laboratory reasons for recommending it.
Using donor sperm doesn't automatically mean ICSI is needed, nor does adding ICSI necessarily improve the chance of success. Your fertility specialist and embryology team can advise whether ICSI with donor sperm is appropriate based on the sample being used and your individual circumstances.
How age and ovarian reserve can influence treatment
Age and ovarian reserve are considered together when deciding between IUI and IVF. Ovarian reserve provides an indication of the number of eggs remaining in the ovaries, while age is an important factor in egg quality and the likelihood of fertility treatment being successful. As ovarian reserve declines and age increases, there may be less time to try several IUI cycles before considering IVF.
There isn't a single age or ovarian reserve result that determines which treatment you should have. These results need to be considered alongside your wider fertility health and family-building plans when weighing the potential benefits of IUI and IVF.
Fallopian tubes, ovulation and fertility history
Because IUI relies on fertilisation happening inside the body, the health of your fallopian tubes is particularly important. At least one tube needs to be open and functioning so sperm can reach the egg. Your clinic may therefore recommend checking tubal health before proceeding with IUI.
Ovulation also affects how an IUI cycle is planned. If you ovulate regularly, insemination may be timed around your natural cycle; in other circumstances, medication and monitoring can be used to support ovulation and time insemination accordingly.
Your fertility history provides another piece of the picture. Previous unsuccessful IUI or other fertility treatment, endometriosis, previous pelvic infections or surgery, and other known fertility factors can all influence whether to continue with IUI or whether IVF should be considered instead. These factors will be reviewed alongside your current test and scan results before a recommendation is made.
Cost, timing and emotional considerations
The practical differences between IUI and IVF can also influence which option feels right for you. IUI is generally less invasive and involves less medication than IVF, and may be an option for people using donor sperm, including single women and same-sex female couples. However, because success rates per cycle are typically lower than with IVF, the possibility of needing several attempts is worth considering when comparing overall costs. Timing can matter too. IUI is a shorter and less intensive process, while IVF requires ovarian stimulation, monitoring, egg collection and embryo transfer.
There is an emotional side to treatment as well. Some people prefer to begin with the less intensive option, while others may feel that repeated cycles and uncertainty would be more difficult. Cost, time and emotional wellbeing are therefore best considered alongside your clinical circumstances rather than in isolation.
When donor choice moves to London Sperm Bank
Once you and your fertility specialist have a clearer idea of your treatment plan, you can start thinking about finding a sperm donor through London Sperm Bank. Your clinic will confirm the type of donor sperm sample required for IUI, IVF or ICSI before you order.
London Sperm Bank provides detailed donor profiles and information about donor screening, including CMV status and genetic carrier screening, to help you make an informed choice. You can then explore available donors through the donor catalogue, select an appropriate sample type and arrange for your chosen sperm to be delivered to your clinic.
Choosing your donor is an important part of the process, but you don't need to have made that decision before discussing IUI or IVF with your fertility specialist. Establishing your treatment requirements first means you'll know what to look for when you begin your donor search.
Questions to ask at your consultation
Your initial consultation is an opportunity to discuss how the differences between donor IUI and donor IVF apply to you. It can help to come prepared with questions such as:
● Based on my fertility tests and medical history, would IUI or IVF be best for me?
● Do I need any further tests before deciding?
● How many IUI cycles might you recommend before reviewing whether to move to IVF?
● If I have IVF, would standard fertilisation or ICSI be recommended, and why?
● What type of donor sperm sample will I need for my planned treatment?
● What should I expect from the treatment timeline?
● How might my plans for future children affect the decision?
● What do I need to do before I can begin treatment?
You don't need to have everything worked out before your appointment – your consultation is there to help you understand your options.

FAQs
Is IUI or IVF better with donor sperm?
Neither is universally better. IUI may suit people with open fallopian tubes and no significant fertility factors, while IVF may be recommended where age, ovarian reserve, tubal health or previous treatment make IUI less suitable. Your fertility specialist can advise based on your individual circumstances.
When might IUI with donor sperm be suitable?
IUI with donor sperm may be suitable if you have at least one open, healthy fallopian tube, are ovulating or can have your cycle appropriately monitored, and don't have significant fertility factors that could make IUI less effective. Your clinic will assess your individual circumstances before recommending treatment.
When might IVF with donor sperm be recommended?
IVF may be considered based on factors such as age, ovarian reserve, blocked or damaged fallopian tubes, endometriosis or previous unsuccessful treatment. It may also be discussed if creating and freezing embryos for future use is important to your family plans.
Is IUI cheaper than IVF?
IUI is generally less expensive per cycle. However, because success rates per cycle are typically lower than with IVF, the possibility of needing several attempts is worth considering when comparing overall costs.
Does IVF with donor sperm have a higher success rate than IUI?
IVF generally has a higher chance of success per cycle than IUI, but individual outcomes vary considerably. Age, fertility health and other clinical factors can all affect your chances, so success rates should always be considered in the context of your circumstances rather than used alone to choose a treatment.
Where does ICSI fit if I am using donor sperm?
ICSI is a laboratory technique used as part of IVF, where a single sperm is injected directly into an egg. Using donor sperm doesn't automatically mean you'll need ICSI. Your fertility specialist and embryology team may recommend it based on factors such as sperm quality, previous fertilisation results or specific laboratory considerations.
Can I try IUI first and move to IVF later?
Yes, this may be an option for some people. How long to continue with IUI before considering IVF will depend on factors including your age, test results and previous attempts. Your specialist can advise when it may be appropriate to review your treatment plan.
Do I need to choose a sperm donor before deciding between IUI and IVF?
Not necessarily. It's usually helpful to discuss your treatment plan and confirm the type of donor sperm sample you'll need first. Once those requirements are clear, London Sperm Bank can support you with donor selection and ordering.
Who helps me choose the donor sperm sample type?
Your LWC clinical team will confirm the sample requirements for your planned treatment. London Sperm Bank can then help you understand the available sample types and support you through the ordering process.
Can same-sex female couples choose either IUI or IVF?
Both IUI and IVF may be options for same-sex female couples, depending on fertility health and how you'd like to build your family. IVF may also be relevant if you're considering shared motherhood (reciprocal IVF), where one partner provides the eggs and the other carries the pregnancy. Your consultation can help you explore which option best reflects your clinical circumstances and preferences.

‘Came here after my treatment at LWC Darlington as I needed to be part of the failed implantation team. The care and communication was second to none. It was everything and more, no question to my coordinator was never too much. They were extremely accommodating when I had to travel from up north multiple times and were always happy to let me have my then 2 year old son come to monitoring appointments. I’m grateful for their expertise in failed implantation as my 3rd egg collection was successful and my fresh transfer worked first time & I’ve just gave birth to my second LWC baby in September 2026. Would always recommend LWC - especially London Bridge / Harley Street.’

‘Going through the IVF journey is always going to be difficult, but the team at London Women's Clinic in Harrow have made it the best experience possible. We've had constant support, check ins, and they have really got to know us and understand our concerns and issues. No question has ever felt like a silly one, and they've been incredibly accommodating when we've needed it too. The whole team have been amazing, but in particular Carla, Ana-Marie and Dr Goswami. We actually relocated away from Harrow 12 months ago, but have continued to go back to the team there as we've built such a great relationship with them, and their support through such a difficult journey has meant the world to us. I can't recommend them highly enough. And probably the important bit for anyone considering going to them - we're expecting our first baby in February!’
